澳洲的Quarantine hotels

閒來沒事,跟各位報告一下澳洲的Quarantine Hotel:

到達澳洲時,先一步一步到行李轉盤領行李、過海關,最後一關即是澳洲政府的Quarantine hotel分派站(不得不提醒一下,澳洲的海關真的超級嚴格,我來的時候被要求行李全部一個一個打開來看檢查,因為常駐醫師需要帶非常多台灣的日用品,去程行李往往超重,而且很多日用品可能都是被禁止的項目,譬如需要攜帶登山杖防身,到了澳洲才發現登山杖也是違禁品之一,因為可能無形中攜帶種子進來)

基本上我們是無法選擇被分派到哪間Quarantine Hotel的,由澳洲政府Quarantine櫃檯分派旅館,

分派好之後,他會請你同一家旅館的乘客聚集在一起,所以你會看到同一班機乘客分別被分派到不同旅館,

然後同一間旅館的,會被『押往』同一台shuttle bus (全程警察護送,你哪裡都去不了),由shuttle bus開到旅館後,警察會引導只能一個人先下車,等一個人進入旅館辦理完check in,才能有第二個人下車 (這點是為了減少大家近距離接觸,這點不得不說,澳洲政府做得非常嚴格,比台灣嚴格太多了)

接下來進入旅館後,警察會在大廳看著你確實進入電梯,到達分派的房間,電梯門打開,雖然沒有警察但是你可以看見走廊到處都有監視器,所以我猜測他們在遠端監督你確實進入房間。

在你走進房間前,旅館非常貼心送上『澳洲政府防疫大禮包』:



基本上就是14天份的醫療口罩,以及提醒你出現呼吸道症狀該打哪支電話,以及防疫期間可以在室內做什麼運動等等,最後甚至還貼心的遞給我一份遲來的晚餐(到達旅館已經深夜11點了)

接下來分享我分派的澳洲旅館房間狀況:


房間可以說是非常舒適整潔,而且重點是三天的轉機期間,防疫旅館完全免費!!全部澳洲政府出錢喔,旅館會提供三餐,甚至在check in時就會給你menu讓你可以選擇每天的breakfast, lunch, dinner,我胡亂點選一通(因為當天到達澳洲,經過戎長的旅途,嚴厲的海關查驗,海關大檢查,加上戎長的隔離旅館分派,到達飯店已經深夜11點,實在沒有精神仔細研究菜單上落落長一大串深奧難懂的英文,基本上菜單上出現的都是專業的青菜專有名詞,肉類名稱,在身心俱疲的狀態下沒有精神再查字典研究了)
分享一下完全不知道在點什麼的三餐照片,如果是英文非常強的台灣醫師,應該可以好好的點一下自己想吃的:






初來乍到澳洲,看到食物的第一眼,老實說非常瞠目結舌,該怎麼說?就是幾乎餐餐都是生冷食物。青菜也都是生菜沙拉,而且還沒有沙拉醬...但是最後兩餐終於看到熱騰騰的漢堡跟炸雞,其實非常感動終於有熱食可以享用!
不過等到來到諾魯的防疫旅館後,你就會知道澳洲的防疫旅館簡直是天堂!!有一種從天堂掉入地獄的感覺~~~正所謂比上不足,比下有餘...尤其最重要的是,三餐也都全部免費遊澳洲政府提供!!
入住期間,澳洲旅館櫃臺會不定時跟你『晨昏定省』,每天打電話到你房間詢問你的身體狀況,轉機前一個晚上還打電話跟我告知,他們查到我的諾魯班機被取消了!!
(真的是太驚嚇了,還好事後打了整個晚上的電話跟大使館、中榮、三總三方聯繫解決了)

恩~~該來考慮一下,下次來跟各位台灣醫師報告諾魯的『五星級』防疫旅館!因為我被分派到的是諾魯全國最高級飯店的防疫旅館唷!敬請期待~~

海外的國慶慶典

 10月份在這個島嶼上,有好多國家同時在慶祝他們的國慶日呢~~

因為其實在這個島嶼上,具有專業技術的人員大部分都來自外國人,

其中當然也包含台灣的醫師、農技團成員,但是更多的是來自南太平洋鄰近的其他島國,譬如:Papua New Guinea, Tuvalu, Vanuatu, Fiji, Kiribati...等等, 也有澳洲人喔 

(全國唯一的醫院RoNH, 醫師組成也是來自Papua New Guinea, Myanmar, Philippines, Vanuatu等等,當然也包括島上唯一的一位台灣醫師)

首先是參加吐瓦魯的國慶慶典

在這場聚會上,除了享用吐瓦魯人民精心準備的餐點之外,更讓人驚豔的是他們的民族舞蹈。男人們互聚集在廣場的中間敲擊物品當做鼓聲,女人們則環繞在四周,穿著草裙,搖曳身姿跳舞,

當鼓聲隆隆作響時,確實有震撼的效果呢!

當人最後賓主盡歡的時刻,吐瓦魯人會邀請賓客上台跟他們一起跳舞,通常台灣醫師是有機會被請上去的喔!這時吐瓦魯人會為上台跳舞的賓客穿上草裙,除了隨音樂起舞之外也可以請旁人幫忙拍照留念喔~~





當然10月還有一個重頭戲~~那就是中華民國的National Celebration Day拉!

今年舉辦在全島最高級的飯店Menen Hotel 餐廳,

除了大使館的大使、參事、秘書、農技團團長及所有成員外,當然台灣醫師也是要一起參與幫忙的,當天諾魯總統、第一夫人及各個諾魯的政要也都會出席觀禮喔~~

這是我個人在海外第一次慶祝國慶日,老實說,身在異鄉,當聽到國歌響起的時候,還是不免慷慨激昂跟著引吭高歌起來,內心真是莫名的感動!!

但與其說是感動,倒不如說是讓人『每逢佳節倍思親』吧~~好想念台灣的一切~~

其實身在台灣真的很幸褔,在這個島嶼上,因為疫情常常運補船中斷或船期不穩定,

我來的第二週就遇到全島買不到礦泉水的狀況,且整整持續將近一週啊,

很多在台灣隨手可得的東西,在這裡都是久久不知道能不能遇到一次,全部食品商品仰賴進口,所以價格自然是水漲船高(這裡物價是澳洲的4倍左右,澳洲物價又約為台灣的2-3倍)~~~

而且有錢還不一定買得到啊!!譬如牛奶、蔬果可能錯過了就要等一個月後的運補船了(不保證一個月後一定有運補船,之前遇到一艘運補船船員染疫,整艘船的貨都沒辦法運進來),所以有時在超商看到好貨,儘管是天價還是要快狠準趕快下手!!

唉~~身在第三世界的不便,真的只有身歷其境的人才能體會@@

國慶慶典大使跟諾魯總統一起切中華民國生日蛋糕!




接著晚會video開始播放台灣對諾魯的各項援助計劃, 其中包含防疫物資援助、醫療包機計畫,看了實在非常感動。其實台灣醫生在島上地位是蠻崇高的,幾乎島上的政要若有身體不適,都是第一個尋求台灣醫師的幫忙呢~~所以這項工作其實也是medical diplomacy的一環。
最後是晚會的抽獎活動囉~可謂賓主盡歡呀!!!
晚會結束了,來張台灣人的大合照是一定要的囉!最後祝福中華民國110年生日快樂!! 
經歷了風風雨雨的上一年,希望未來的一年天佑台灣!

一波三折的諾魯行

這是我本人第一次來到諾魯這個全世界第三小的國家~~~

出發前因為受到Covid-19疫情的影響

華航、長榮班機相繼被取消....

最誇張的是,原本預計9/14要搭乘長榮航空,當天我已經在起飛前2.5小時抵達機場,結果到了長榮櫃台被通知小姐您的班機被取消了啊!!!

緊急聯絡旅行社後,旅行社說沒接到任何班機取消的通知~~

進一步詢問長榮航空,他說因為是透過旅行社代訂,航空公司只會通知旅行社,不會通知我本人!!並且問到班機是起飛前三天突然取消的!(算了一下,我是禮拜二的飛機,等於禮拜六航空公司突然取消,又剛好遇到六日兩天休假,如果禮拜一旅行社上班沒有特別follow就miss掉了)

經過緊急與中榮、台灣駐諾魯大使館聯絡後,

最後一波三折,於9/22搭乘長榮航空『商務艙』先到澳洲布里斯本,再從布里斯本轉機到諾魯

(沒錯!因為當前澳洲日趨嚴格的邊境政策,每架飛往澳洲的班機只能有15個乘客!!以航空公司角度飛一趟虧一趟,所以現在基本上買不到任何經濟艙了,飛機上商務艙乘客比經濟艙多很多,由此可見航空公司的銷售策略也跟著轉向,而且班機紛紛取消....)

當然我也在一陣驚嚇後,又提著行李返回我溫暖可愛的家,多了一個禮拜好好與家人相擁道別的寶貴時光

所以各位台灣準備前往諾魯執行任務的醫師們,行前務必『再三』提醒旅行社幫忙追蹤班機取消狀況喔!即使起飛前班機都可能被取消~~

接下來就是好好享受華航商務艙的艙內設施與美食~~~








當然這是最後幾天可以好好進食的狀況,所以各位在商務艙吃飽一點囉!

「螞蟻世界的權力戰爭」

「螞蟻國的權力戰爭」
Ant Game of Throne

南太平洋的未知世界裡
一個叫做諾魯的遙遠島上
藏著神秘數字217的異次元空間
住了來自福爾摩沙的白袍巫師
(巫師平時的工作我們另篇敘述)

這個神秘的時空洞窟裡
還另外住了幾個種族,
幾個世紀以來
偶爾會造成巫師的困擾:
小矮鼠一族的鼩鼱、蟲族之中長得很像台灣大蟑螂但是笨很多的蜚蠊、以及
其實看起來就像是螞蟻的三種不同品種螞蟻家族🐜:
按照三個螞蟻家族的規模大小
排列介紹如下

黑頭慌蟻 ( Tapinoma melanocephalum ) 

狂蟻 ( Paratrechina longicornis )

小黃家蟻  ( Monomorium pharaonis )

為什麼要特別介紹他們呢?
因為 小矮鼠偶爾會出現到處啃咬挖洞破壞珍貴物資與巫師得來不易的各種魔法裝備!
不過是否因為島上野貓家族開始增加之後,就很少看到小矮鼠家族出沒了~
至於不討喜的蟲族蜚蠊,原本每天都要被迫打怪數次,自從巫師把天花板上的空調維修孔直接用膠帶整個封起來之後就大量減少,可見蟲族應該是從這個維修孔突破防線入侵的!
但是 不管有沒有食物,
螞蟻三家族總是會在洞窟的地上桌上牆壁上甚至天花板上到處爬來爬去,
甚至肚子餓沒有食物吃的時候
 還會莫名其妙跑來咬白袍巫師!
真的是品行不良的家族!
後來經過仔細研究發現
「三大螞蟻家族的窩 」
其實就在217洞窟的牆壁裡!
因此 不管怎麼消滅屠殺螞蟻家族
都會有另外一個螞蟻家族趁機崛起彼此互相消長~
簡單講就是
「削弱其中一種螞蟻家族之後,
另外一種就會增加起來」
基於這個理論基礎
我們先分析一下
這三種不同螞蟻家族的習性:

「小黃家蟻」
就是我們俗稱的小紅螞蟻,
跑得慢+會咬人!最令人討厭!
巫師一開始發現此家族時
就直接使用劇毒魔法撲殺整窩!
目前已經幾乎看不到此家族出沒了~

「黑頭慌蟻」
超級小的螞蟻,捏死會臭臭的!
綽號 嗜糖蟻,巫師一開始撲滅小黃家蟻之後大肆崛起的螞蟻家族!
台灣本島的種類不太咬人,
但是不知道為什麼諾魯的偶爾會咬人!
非常喜歡到處爬+到處鑽!
見縫插針跑進各種食物的包裝裡面亂咬一通,非常不討喜的個性!
跟其他兩種螞蟻家族也是水火不容,每天在巫師的地盤上打架爭奪食物與空間~

「狂蟻」
是唯一個性比較好一點的家族,
就是台灣俗稱不會咬人的黑螞蟻,
名稱聽起來很狂,其實戰鬥力很弱~
個性非常敏感+神經質,
風吹草動緊張起來跑得非常快!
就像是「發狂」了一樣~
非常害羞+絕對不會咬人!
因為狂蟻家族普遍攀爬能力不好,
不太會像其他兩種螞蟻家族一樣沒禮貌
常常沒事無聊跑到沙發上桌上來打擾巫師吃飯!

然後 巫師觀察了一下這三大螞蟻家族的互動與歷史文化之後,就召喚Google魔鏡搜尋了一下,看看有沒有什麼好辦法可以處理這三個螞蟻家族令人困擾的問題?
過去螞蟻家族百年來的戰爭與互鬥,
小黃家蟻只有對付巫師比較厲害而已~
與其他兩種螞蟻1:1幹架都是輸!
搶地盤搶食物都輸!
目前已經差不多消失了~
結果黑頭慌蟻家族大舉攻城掠地,
讓巫師覺得非常煩⋯⋯
因為慌蟻家族爬得到處都是!
還會從天花板空降掉下來巫師身上!

後來巫師也嘗試用魔法撲殺慌蟻,
並且開始默默用食物碎屑贊助一下狂蟻家族,
結果其他種類的螞蟻
竟然大量減少 甚至幾乎消失了!
這個結果 真是令人震驚啊!
因為這些狂蟻沒事就會在地板上瘋狂的衝刺巡邏,結果竟然連蜚蠊家族都減少了!
甚至發現 狂蟻不太喜歡吃麵包屑,
反而比較喜歡群起攻擊那些受傷的蟲族,難怪網路上描述有些人會養這種不會咬人的螞蟻在家裡面驅逐其他小蟲,真的是非常有趣的一件事情啊!

最近甚至發現
狂蟻之中誕生了一位「龍后」,
幾百隻的狂蟻竟然制伏了小壁虎龍!
不知道狂蟻們沒有勇猛的大額獠牙是怎麼辦到的?巫師這兩天週末休息就看著幾百隻的狂蟻戰士團簇擁著龍后與她的龍,來來去去炫耀非常威風!甚至 還到處攻擊蟲族蜚蠊家族並且運走所有的屍體!(變成異鬼?)

總之 經過多方征戰之後,
如果沒有辦法斷絕所有困擾的話,就幫助最討喜的那種黑螞蟻,他們會全天候24小時攻擊其他令人討厭的小蟲蟲,還會順便清除一些小屑屑,真的是很不錯的種族統戰組合策略!

tPA治療STEMI

急性心肌梗塞Tx
在台灣最近十幾年來
就是衝PCI治療
快快做診斷 早早衝治療
當初什麼MONA t-PA早就快過時了~
但是在諾魯共和國這個島上
糖尿病胖子一大堆
AMI患者是沒有PCI這種選擇的!
原本也沒有t-PA這種選擇!
只能禱告上帝保佑沒事平安就好~
自從2021年初RoNH藥局購買了t-PA之後,感謝Dr.翁Taiwan心臟專科醫師協助製作STEMI標準治療流程與病患須知簡章(電子檔置放於ER)供當地醫院工作人員使用!(ER有ECG機器)
